*If referrals are required it is your responsibility to obtain one from your primary care physician and one from infant's pediatrician before your visit. They can be faxed to our office at 516-829-0981. Thank you.

If Dr. Macaluso does not participate with your insurance plan, we can fill out a Health Claim Insurance Form for you to mail to your insurance company for you to be reimbursed. As Dr. Macaluso is a physician the visit should be covered. You can call your insurance company before the visit to confirm and be sure to mention she is a physician.
