What to bring to your first visit:

1. Your intake form: You can download, fill out and bring in or fax it to us. The fax number is 516-498-2301. Or you can wait until you get here to fill it out.

2. Your insurance card and your baby's insurance card, if they have one already. Most of our babies don't yet have one of their own.

3. A hungry baby. Please try to feed the baby about an hour before our visit (or longer if you have an older infant). This way we can observe a feed and work together to optimize latch and positioning.

4. Anyone else you want to bring with you. Your husband, parent, in-law, other children, baby nurse, doula, friend? It's always great to have everyone educated and supportive of your decision to breastfeed.

5. Anything else you want to discuss concerning you and your baby. A list of questions, charting of feeding/infant urination/stooling, your breastpump, breast shields for fitting, supplements or medications, bottles, nipple shields or feeding devices.

6. Your copays for you and your baby, cash or check is accepted. Thank you.
