1. You will need to check in at the front desk
2. You will be asked for which Dr. Macaluso you are here to see
3. The receptionist will then ask for your insurance card and if you brought or faxed in the intake form. If you have not then she will give you one to fill out.  If you have a fax machine you can fax it to us at 516-829-0981.
4. She will also ask if you have any CD’s, bloodwork or other documents that you have brought in for me to review
5. You can have a seat while you fill out the forms. After you are done, she enters your data into the computer system. If you filled out the forms at home the receptionist will enter the data into the computer system as soon as you give them to her. If you faxed your forms in then all she needs to do is enter your insurance information.
6. I will come out to bring you into my office after your chart is all set up.
7. We will talk about the problem you came in for and get your full history.
8. Next I will do a neurological exam to help figure out what part of your nervous system is being affected.
9. Then we will talk about what was found.
10. I will tell you if there is any necessary testing that needs to be done to help guide the management of your case or to help reach a diagnosis.
11. You are encouraged to ask questions about anything that you are unsure about. Good communication is crucial to helping anyone get better.
12. You will then be taken back to the receptionist where you will be given instructions for any testing that needs to be done. You will also be scheduled for follow-up. You will also be asked for your co-pay. Cash and checks are accepted.
