This is what you should bring to your first visit:
- if you are coming in for a neurological problem other than multiple sclerosis please download, fill out and bring in or fax the general intake form
- if you are coming for multiple sclerosis please download, fill out and bring in or fax the MS intake form
- note that if you have a fax machine you can fax in your intake form (either general or MS) so that we can enter your data before you come in which will decrease your waiting time! The fax number is 516-829-0981. Address the fax "Attn Mercedes"
2. your insurance and drug cards
3. any recent bloodwork
4. any imaging burned on CD
- if you have had a MRI or CT scan for the condition that you are coming in for it would great if you could bring in those studies on compact disc
- since most imaging is in digital format now the place where you got the study done has the ability to burn it on CD. If you are told that it cannot be done, ask to speak with the radiologist.
5. anyone you want to bring with you
- this includes family, spouse or friend
6. your copay
- cash or check is accepted
